On February 19, 2016, at approximately 8:15 a.m., an employee was cutting 6-foot lengths of fiberboard down to 43 inches using a miter saw. While holding the material with her left hand, she used her right hand to lower the blade of the saw. The blade made contact with and amputated her left thumb tip.
